Breast Implant Placement

The best breast implant placement depends on the size of the breast implants, the individual anatomy and other factors related to the goals and expectations. Breast implants can be directly under the natural breast tissue, under the pectoral muscles or behind the breast tissue and partially under the pectoral and other chest muscles. The shape of breasts after implant enlargement or Augmentation Mammoplasty is in large part determined by the relationship of the implants to the pectoralis muscles of the chest wall. Implants can be either above the pectoral muscles, or beneath the muscles. The route of placement of implants under the muscle determines whether the implant is totally or partially covered by muscle. A number of consequences may result according

Sub-glandular implant placement places the saline or silicone breast implant under the breast tissue, but above the muscles in the chest. Partial sub-muscular implant placement places the silicone or saline breast implant under the breast tissue, and partially under the pectoral and other chest muscles. Complete sub-muscular implant placement places the breast implant under the pectoral and chest muscles.

Breast Implant Incision Sites

The most common breast implant method of insert is through a small 2 to 3 cm incision at the areola border. The transition in skin color between regular breast skin and the darker areola skin color is the perfect place to hide a small scar.

Another common site to insert breast implants is the fold under the breasts. As the breasts enlarge, the breast tissue will typically fold over at the bottom, thereby covering any visible scars in the standing position. When a patient lies down the scars may be visible.

Large breast implants

Large breast implants add generous size and contour to any woman's frame. Because big breast implants come in a variety of sizes, and because every size will produce different results on different women, careful thought and consultation with a highly qualified breast implant expert are necessary to achieve optimal results. Selecting the proper size of large breast implants to achieve desired results requires careful consideration. While 600cc to 700cc breast implants will bring most breasts to at least a DD size, for many women this is not enough. Women who want to make a statement select 750cc, 800cc, 900cc, and even 1000cc breast implants. If one is considering large breast implants, choose a breast implant surgeon who has extensive experience placing large breast implants. The doctor should be able to show before-and-after photographs of women who have had breast augmentation to size DD, as well as women who have had 800cc to 1000cc breast augmentation; so that can get an idea of the change that large, breast implants will bring to the body. The doctor should also fully describe the potential risks and complications from breast implants, and the physical impact that big breasts implants have. Only after fully understanding how the look and the physical considerations of the larger breast implant sizes should the patient proceed with surgery. Increasing breast size is the primary reason most women choose to undergo breast enhancement surgery. Large breast implants, such as those used in DD cup breast augmentations, are ideal for women who desire a dramatic change in the contour of their bodies. Large breast implants have advantages and disadvantages. Sometimes a woman seeking large breasts will discount the downside of big breast implants. However, careful examination of the advantages and the disadvantages will help make an informed decision about what size will be most appropriate for in the long term. The cosmetic surgeon can provide information that is specific to each body and patient goals.

Capsulectomy

A capsulectomy is the surgical removal of capsular contracture around a breast implant. Capsular contracture or the formation of tight scar tissue around the implants occurs in up to 10 percent of cases within the first 10 years of having the implants. Research is on going for the specific cause of capsular contracture. Some studies suggest that a systemic inflammatory or infectious process triggers the immune system and cause the inflammatory cells in and around the implant capsule to make more scar tissue around the implant, causing thickening and tightening of the capsule. Routine activities such as a tooth cleaning, which releases bacteria into the bloodstream and can elicit a short-term immune response similar to the common flu, can sometimes trigger a heightened immune system response. The incidence of capsular contracture remains so low that patients do not need routine prophylactic antibiotics for tooth cleanings, but further research may soon prove otherwise.

Four Stages of Capsular Contracture

There are four grades of capsular contracture. The Baker Grades I through IV are: Grade I - the breast is normally soft and looks natural; Grade II - the breast is a little firm but looks normal; Grade III - the breast is firm and looks abnormal (visible distortion); Grade IV - the breast is hard, painful and looks abnormal (greater distortion).
